Submit an expense payment or reimbursement form.

For an Expense/Reimbursement Form click here to print a copy or pick up a form from the main Office; forms are located next to the mailboxes. Make sure to attach original receipts to the form.  Place completed forms in the mailbox of the staff person formally aligned with your activity/group for approval and processing.

Those submitting an Expense/Reimbursement form for a recognized UUAA Social Justice Group, please place the completed form into the Social Justice mailbox located in the main office.
